POSITION: Home & Community Care Nurse Department: Health Center

Position Summary:
Reporting to the Director of Health, The Home and Community Care Nurse (HCCN) functions as a member of the HCC team to provide home care and chronic care services. The major goal is to encourage independence and family support through the provision of education, monitoring, intervention, and support.
Duties and Responsibilities:
• Fluency in Ojibwa considered an asset
• Active practicing membership of LPN, RN, BN register of the College of Registered Nurses of Manitoba (CRNM).
• Current CPR/First Aid certificate
• Adheres to the “Regulated Health Professions Act”
• College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses
• Chronic Disease Registry system
• Scope of practice as documented in the Regulated Health Professions Act
• Nursing care knowledge related to health assessments, diagnostics, and referrals.
• Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace
• Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) and Other legislated acts
• Assesses, develops, delivers and evaluates, using models of health promotion and best evidence.
• Uses culturally sensitive community development strategies to partner with community members to identify and address their shared health concerns. Builds on existing community strengths to maintain and enhance good health practices.
• Advocates for individual and system wide access to needed programs to deal with health issues.
• Collaborates with individuals, families, groups, and community to identify potential health risks and make choices for appropriate health management strategies.
• Pursues professional development relevant to community nursing practice and utilizes self-reflective practice to evaluate own care.
