POSITION: Home and Community Care Coordinator

Under the direction of the Home Care Nurse, this position delivers intake, assessment, monitoring and supporting to the clients of the Home and Community Care program. This position also provides direction to and supervises the homemakers and home maintenance workers.

POSITION DUTIES AND RESPONSIBILITIES:
• Conducting client intake and assessment visits, using appropriate assessment tool and documenting information on
the form
• Preparing client care plan for review by the home care team
• Organizing home care services in the home in an innovative and adaptable manner to manage the client’s needs
successfully
• Providing an objective, non-judgmental approach to client needs
• Maintaining accurate client files
• Setting up and working with the home care team to review and approve client care plans
• Ability to train new employees and provide performance reviews.
• Able to motivate self and workers in specific activities

QUALIFICATIONS:
• Grade 12 Diploma or equivalent
• Nursing or Health Care Aide background
• Be in good physical condition to meet demands of the job Knowledge,

SKILLS AND ABILITIES:
• Excellent Communication and interpersonal skills
• Ability to work independently.
• Time management and ability to prioritize workload.

CONDITIONS OF EMPLOYMENT:
• Access to reliable transportation with valid Driver’s License
• Clear Criminal Record and Vulnerable Persons check and Child Abuse Registry check.
• Knowledge of aboriginal culture /speaking and understanding Ojibway would be considered an asset
• Must be willing to submit to random drug testing.
