POSITION: Community Health Representative

Under the direction and supervision of the Community Health Nurse (CHN) and the Director of Health, the Community Health Representative (CHR) is responsible for participating in the delivery of high
standard community health, treatment and surveillance programs in order to provide quality health prevention and treatment in the community. As a member of the health care team, he/she is also responsible for liaising between families, patients, the community and health care providers to ensure patients and families understand their conditions and treatment and are receiving appropriate care.
The incumbent will also educate community by hosting various workshops on general health.
DUTIES AND RESPONSIBILITIES
• Must have full knowledge of PHIA – Personal Health Information Act and Confidentiality
• Plan weekly work schedule, set goals and objectives
• Must be computer literate
• Keep records and provide activity reports – staff meetings
• Must complete client documentation through charting and filing
• Encourage and support community initiatives through attending various community
meetings (band, health, program managers)
• Assist clients and community members with medical forms etc.
• Order medical supplies, example; hearing aids, glasses, etc.
• Organizing by planning workshops and annual Health Fairs
• Organize training e.g.: CPR, First Aide, food handlers, logistics, etc.
• Statistics (e.g. attendance at community meetings, number of people immunized, etc.)
• Coordinate transportation for clients (when needed)
• Assist CHN with Chronic Disease Care, Diabetes and Hypertension education
• Assist CHN with Immunization Clinics
• Accompany CHN on home-visits
• Assist in developing a Health Care Plan to meet client’s needs
• CHR is responsible for developing and maintaining respectful professional working
relationships with clients, families, and communities.
• CHR is responsible for any additional duties as assigned.
Required Knowledge, Skills, and Abilities:
• Minimum of 1-2 years experience in a clinical nursing or community health setting
• Computer literacy, including effective working skills of MS Office Software products, email and
internet search engines
• Must understand programs, e-Chart, MUSTIMUH and SOAPIE
• Ability to use a blood pressure cuff, thermometer, oximeter, and demonstrate the usage of
glucometer.
• Knowledge of Indigenous culture and political structure
• Experience working in a culturally diverse environment.
• Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace
• Hazardous Material Information System (WHMIS)
• Strong communication and interpersonal skills
• Excellent teamwork and team building skills
• Effective verbal and listening communication skills

• Time management skills
• Must have valid CPR and First Aide
CONDITIONS OF EMPLOYMENT:
• Must have valid Drivers License
• Must have reliable vehicle as travel is required
• Clear Criminal Record and Vulnerable Persons check and Child Abuse Registry check.
• Must be willing to submit to random drug testing.
